Docket® calculates Immunization Status based on a.) which shots are reported to the state and b.) data which Docket® receives from the state. Always be sure to consult with a trusted healthcare professional before receiving a new vaccine. Not all immunization statuses are supported in every state.
You can access your personal and/or family immunization records using Docket® if you have a valid phone number and/or email address on file with your state’s immunization registry (IIS). Your name, date of birth (DoB), and legal sex in Docket® must match your state record exactly.

How does Docket@ work? Residents of Alaska, Idaho, Minnesota, New Jersey, Utah, and Wyoming: you can access your personal and/or family immunization records ifyou have a valid phone number and/or email address on file with your state's immunization registry (IIS).
